I can tell you that hyaluronidase (Vitrase, Hylenex) is an enzyme specific only to hyaluronic acid in the same way that deoxycholic acid (Kybella) is specific to fat. What is intra-lipotherapy?

I leaned back, exposing the vulnerable area under my chin, and braced myself for the prick of the needle. I clutched an ice pack, ready to apply it, and looked around, my heart racing. I was about to have vials of synthetic gastric acid injected into my double chin, which I hoped would erase it forever.

The drug being developed to treat my “submental fat” (the pocket of fat under the chin) is the new cosmetic injectable Kybella, which promises to dissolve fat cells through a series of injections. There is very little downtime, it is non-invasive and non-surgical and there is little risk. The drug is the first (and only) injectable designed to contour the dreaded double chin.

Approved by the FDA in 2015, Kythera Biopharmaceuticals’ (now acquired by Allergan) drug hit the market as an alternative to liposuction or surgery, using a synthetic version of deoxycholic acid — a salt found in human bile and involved in fat digestion supports. destroy fat cells. According to Conor Gallagher, Allergan’s executive director of medical affairs, European scientists began using a compound called phosphatidylcholine to break down fat for medical reasons in the ’60s. After a patient suffers trauma, perhaps from a fracture, tiny lumps of fat can enter the circulatory system. “And often they end up in the lungs and can cause breathing problems,” Gallagher said. “So they’re looking for a way to destroy fat, fix it, or treat it. They used phosphatidylcholine as a potential agent to try and break down that fat.” Cut to Europe in the 1980s when cosmetic researchers started looking at the compound and thought, if it can break down fat in the vessels, maybe it can elsewhere Dissolve fat, Gallagher said. As I hit my 30s, I started to be a little bothered by my so-called double chin. While not particularly noticeable, this small pocket of submental fat has been a part of my profile for as long as I can remember; it’s genetic. I’ve often wished it was gone, but never thought of a surgical solution. That seemed like an extreme measure for what is just a nuisance in my life. I have never seen myself get any type of cosmetic procedure. Then I saw a magazine ad for a new injectable, non-invasive double chin treatment.

I was able to try Kybella courtesy of the Beverly Hills Cosmetic Spa Alchemy 43. Concerned about being injected under the chin many, many times (the average patient receives 20 or more injections per treatment), I arrived at the “beauty lab”. It’s the kind of chic place one would imagine visiting for such a procedure, with futuristic light fixtures, high-end products on display, and luxe gold accents. It feels less like a medical setting and more like an upscale boutique, complete with a velvet couch. The young women who cast Alchemy 43 wear flawless makeup and perfectly smooth faces—a look I don’t see often in my life as a homemaker and writer on the laid-back east side of Los Angeles. Other services offered by Alchemy 43 include wrinkle relaxation, fillers, modeling and lash enhancements and I can imagine for a minute what it would be like to have the time and money to get all of these treatments. I would never be able to keep up; I don’t even get my hair cut more than once or twice a year.

Owner Nicci Levy told me that the spa visits about 30 clients a month for Kybella and that all clients have seen a positive result. “People are very interested in this treatment,” she said. “We’ve seen amazing results.” My injection expert (or “alchemist” as she’s called in Alchemy 43) Katie Saliman is a tall, blonde nurse with a strong chin. Saliman told me she loves feeding Kybella because of its “wow factor.” “This product can absolutely transform your profile and really improve the shape and contour of your jaw,” she said. I appreciated their enthusiasm.

Saliman was involved in the clinical trials for Kybella; Her experience eased my fears of being poked in the throat with needles full of acid. In studies, about 4 percent of subjects had nerve damage in their lower jaw, and this was my biggest concern prior to the procedure. I asked about the possibility of hitting a nerve, and Saliman assured me that none of her patients had. Even if it did happen, she said, I’d have a crooked smile, but it was only temporary — the damage usually wears off after a few months. I guess I could handle nerve damage, I thought wryly.

Saliman took a series of high tech 3D photos, some “before” shots and applied numbing cream under my chin. Then she applied a temporary tattoo dot grid; a map of the injection sites. With a little help from a similarly beautiful assistant, Saliman began to poke. I felt each needle go in, but it wasn’t unbearable. If I could survive labor and root canals I figured I could definitely handle it. I began to feel a burning sensation as the medication took effect and I cooled my chin when Katie paused for a new needle. In total I had about 30 injections from three ampoules of Kybella. Although there were many, the recordings themselves only lasted about five minutes. Saliman recommended I do two more treatments about six weeks apart. “Everyone needs two to six treatments, and we’re big proponents of ‘treat to complete,'” Levy said.

For each treatment, the injector uses between one and three ampoules of Kybella, depending on how much submental fat you have. According to Deanne Mraz Robinson, a dermatologist and director of the Connecticut Dermatology Group’s Body Sculpting Center, the cost of a single treatment ranges from $800 to $1,400, depending on how many vials you need and your doctor’s or local pricing Doctor’s Spa. Kybella’s cost can exceed that of liposuction (which generally ranges from $2,000 to $5,000), but many customers are willing to pay a higher price to avoid going under the knife. “People are excited that there is an option to treat their double chin,” Mraz Robinson, one of the investigators in Kybella’s pivotal study, told me. “Previously, the only procedure available to treat submental fullness was surgical liposuction, which is an invasive procedure with its own risks and benefits.”

Saliman told me that I’m a great candidate for Kybella because my submental fat is genetic and not weight related. And I’m 34. Younger clients usually do well because they have better skin elasticity and are less likely to have excess, sagging skin under the chin. “It’s important to make sure the fullness under the chin is from fat under the skin and isn’t caused by other conditions like skin laxity,” explained Mraz Robinson. “Submental fat can be seen across a wide age range, as factors such as genetics, hormonal status, and weight can affect the amount of fat under the skin.”

An interesting trend has emerged in market research and studies, Gallagher told me: Kybella brings into play a whole new group of injectable consumers. Approximately 45 percent of the people who receive Kybella are new to aesthetic treatments – they are people who have never been to a cosmetic or aesthetic doctor’s office before. (I’d be included in that group.) And the process attracts an unlikely clientele: men. “Many men who would not previously have considered cosmetic procedures come to Kybella to sharpen their jawline,” noted Mraz Robinson. Levy also told me that she has noticed a younger group coming to “sculpt” procedures like Kybella. Millennials seem to be interested in injections.

I’ve never felt quite like L.A. driving through Beverly Hills with a medical compression bandage around my head. My chin was quite sore for about two hours after the injections, but I started to feel better pretty quickly. As predicted, the injection area began to puff up like a bullfrog. I slept with my head propped on two pillows, the bandage around my chin.

Swelling typically peaks 24 hours after treatment and can last from two days to three weeks. It took me about three weeks to get back to baseline. But that’s normal, according to Saliman. I was told not to drink alcohol or take painkillers 48 hours before my injections and luckily I only got a tiny bruise (the day after the treatment). The inflammation wasn’t comfortable, but the swelling means the drug is working to dissolve fat — no pain, no gain. The swelling is caused by the active drug breaking down the fat cells, and some of the volume can be attributed to the solution itself. Sometimes I’m in disbelief that I actually went through something so vain, and sometimes I’m proud that I actually withstood all those needles. “You tolerated it like a CHAMP!” Saliman emailed me afterwards. This makes me hard, which is weird and awful because we’re talking about getting injectable cosmetic treatments in Beverly Hills. But I’ll take it! I am a true warrior.

Gallagher confirmed that swelling is a good thing. And swelling after subsequent treatments will not be as extreme. You may also experience bruising, pain, numbness, redness, and hardened areas (which may feel like lumps or small pea-like bumps due to swelling) in the treatment area. All this, he tells me, should be temporary. I initially had numbness and a bit of pain, but my side effects were bearable.

The process can be relatively slow, with results taking weeks or, more commonly, months. This can be discouraging, especially for people who are initially unsure about their chin. The good news is that those fat cells don’t come back once your months-long process is complete. There is no maintenance or future costs to consider. “You’re taking away those fat cells, they’re not going to recreate themselves,” Gallagher said. Safety studies have been conducted on the long-term results, and “generally, between 75 and 90 percent of patients maintain their response for up to five years.” Gallagher also noted that after the second treatment, most patients really turn the corner and have a noticeable one Notice a reduction in fat.

I haven’t seen my full results yet, but in more than 20 studies, nearly 70 percent of the thousands of patients reported physical and emotional improvements. That sounds promising. Two US studies submitted to the FDA showed that nearly 80 percent of people treated with Kybella noticed a reduction in their submental fat and thus had greater satisfaction with their appearance. And doctors are also rejoicing at the advent of this drug. “Doctors have really enjoyed the precision and potency of Kybella, I would say,” Gallagher noted. “It’s doing what it’s going to do… it’s going to kill fat cells wherever it’s injected.” Well, wherever it is injected within reason.

Kybella could be used on various other small areas. From a clinical perspective, Allergan examines the “bra fat” area – the roll of fat that may be present under the bra line or between the bra and the armpit – and the cheek. “Sometimes it’s just skin laxity, but there are times when that’s also due to fat … so we’re certainly actively considering going into a clinical program there and looking at cheek fat,” Gallagher said. Mraz Robinson told me that “Kybella will likely be approved for use on body areas to reduce subcutaneous fat,” and my injector agreed, noting, “This type of practice is already being used in the front bra fat area and upper knees.” So while larger areas have been ruled out (a swollen belly, for example, would require too much product and too many needles), investigation is ongoing to see if other small love handles can be helped with Kybella. Studies would be needed before FDA approval for use in these other areas.

I didn’t see much of a difference after my first treatment, but deoxycholic acid is currently popping fat cells after my second treatment which was about two weeks ago. I’m just noticing a reduction. The second treatment is often the turning point, the experts tell me, and Saliman is optimistic I won’t need a third try. I had lidocaine injections for anesthesia plus the Kybella shots for the second treatment and the increased volume caused more swelling. Overall, the second treatment was a bit more painful than the first and I bruised more. One thing I noticed while examining the profile photos on my “Kybella journey”: Maybe there wasn’t a big problem from the start! Perhaps this softness in my profile is more common than I thought, and not the “double chin” I was imagining. But does Kybella work? I will be more than happy to have a slimmer chin.
